Electronic Communication & Privacy

The Mole Clinic (Dr. R.M. Khan) collects and uses your personal health information to provide care, arrange your appointment, communicate with you, and administer our practice. We handle this information in accordance with Ontario's Personal Health Information Protection Act (PHIPA).
By completing this form, you may be giving us an email address and/or a mobile number so that we can reach you electronically. Please understand:
What we use electronic messaging for
We use email and text messaging mainly for administrative purposes — appointment booking, reminders, confirmations, forms, and general instructions. Email and text are NOT suitable for medical emergencies or urgent problems. If your concern is urgent, call the clinic; if it is an emergency, call 911 or go to your nearest emergency department.
The risks of email and text
Standard email and text messages are not encrypted and are not a fully secure way to send personal health information. There is a risk that messages could be:
- sent to, or seen by, the wrong person;
- intercepted while travelling over the internet or an unsecured/public Wi-Fi network;
- read by others who have access to your device, email account, or phone; or
- lost or exposed if a device is lost or stolen.
We take reasonable steps to protect your information, but we cannot guarantee the security of messages once they leave our office or reach your device.
Your responsibilities
Please give us only email addresses and phone numbers that are private to you, tell us promptly if they change, and keep your own devices and accounts secure. Replying to our messages, or continuing to communicate with us electronically, tells us that you accept these risks.
Your choices and your consent
Giving consent to electronic communication is voluntary. You may ask us to contact you by phone or mail instead, and you may withdraw your consent to email or text at any time by telling the clinic — this will not affect the care you receive. You also have the right to access your health records and to ask us to correct them.
